Special Chiefs Assembly 2012 - Registration

Please complete the SCA registration form and return by fax to the Assembly of First Nations, attention Regina Toulouse, at (613) 241-5808. Cut-off date for Registration Forms and payment to be received in the AFN Office will be November 27, 2012 ~ after this date, please register on-site with your payment.

Pre-registration will be held on Monday, December 3, 2012 from 4:00 – 7:00 p.m. at the Hilton Lac Leamy – Convention Level. For planning purposes and to avoid line-ups, all delegates are encouraged to pick up badges, USBs and kits on the evening of December 3, 2012.

Please note that registration fees are non-refundable after November 27, 2012. An administration fee of $50.00 will apply for all cancellations.

All participants that have pre-paid registration by November 27, 2012 will receive details by e-mail; in addition, all pre-registered and pre-paid delegates will be eligible for the PRE-REGISTRATION DRAW (IPAD2) which will be presented at the Assembly.

To facilitate registration, all participants and groups are encouraged to pre-register and pre-pay.
